Dear Wakefield Parents:
Please read the information below concerning Sports Physicals coming soon at Wakefield on June 11. Please make sure you use our physical form whether your student gets a physical at Wakefield or at their own doctor. Registration and link to the physical form are listed below.
It’s that time of year when coaches are having interest meetings for next year’s athletics. The good news is students can start turning in sports physicals NOW for next year!! Make your appointment now, and get it taken care of so you don’t have to worry about it over the summer.
If you are interested, we are offering physicals in the Wakefield clinic (room A-123) on Tuesday, June 11. One of the doctors is Dr. Najarian, Wakefield’s Team Physician, an orthopedic surgeon with INOVA Sports Medicine (offices in Ballston and Annandale).
Cost is $25, checks made payable to WHS.
HOW TO REGISTER:
You can email Ms. Kigin at firstname.lastname@example.org to sign up for a spot or you can click on this link for a VERY basic registration form, or copy and paste this link: https://forms.gle/RYFA2HmanA5MbtQm9
Registration for Physical Exams
Choose the time slot you are available for. You’ll receive an email to let you know if you have a confirmed appointment at that time or if you’ve been placed on the waiting list. All physicals will be performed in the clinic, Rm A-123, and cost is $25 (cash or check to WHS).
Appointment slots (5 athletes per slot) will be given between 4:15 pm and 7 pm: if your child fails to arrive for their allotted time, they will have to wait until the end to be seen by the doctor. If the all the spots are filled, we will start a waitlist. You will be notified via email with your confirmed appointment time. (The email will be sent during normal working hours by a real live person, so please be patient!)
WHAT TO BRING:
Please download the 2019-2020 Physical packet from the Wakefield website, or have your child pick up a print copy from the Activities Office (E-117). All forms MUST be signed in order for the doctor to perform the physical (essentially it is the “permission form” for him to perform the physical)
Please bring payment ($25, check to WHS, or cash).
1. On the day of the physicals, the doctors are coming from their various practice locations or surgery, so please understand they may be delayed in arrival, so there may be a backlog; bring something to read, knit or watch (or go for a walk on our lovely track!). You may also use the time to commiserate with other parents about the joys of teenage children while you wait! 🙂
2. The doctor is performing the physical for athletic participation only. In some instances, the doctor will require additional information prior to clearing your child, which often requires further evaluation from either the child’s primary care physician, their treating physician for a previous condition, a cardiologist, etc. If this is the case, the students will not be allowed to participate until the doctor’s recommendations are met.
If you have any questions, please feel free to email Ms. Kigin, email@example.com . Yay for getting this out of the way before summer! 🙂
